The high incidence of schizophrenia causes a greater burden on society and family. In the past of half century, with the widespread use of schizophrenic drugs,the prognosis of clinical schizophrenia has been substantially improved after following the principles of early, effective and maintenance therapy.In this overview,the clinical characteristics at different stages of disease progression with the recent advances in drug therapy of schizophrenia was combined, to introduce clinical selection of schizophrenic drugs.

ABSTRACT

Depression is a common mood disorder, with a high incidence and prevalence, while the overall diagnosis and treatment rate are low.Even if patients attained the initial clinical cure, there is still a high risk of depression recurrence, of which a great part may turn into self-mutilation or even suicide.Taking antidepressants as the most preferred treatment of moderate and major depression is recommended by almost all clinical treatment guidelines at home and abroad currently.At the same time, the concept of depression treatment is changing gradually from the initial single-mode drug therapy for symptom control to a comprehensive, individualized, quantitative treatment model. Promising psychological treatment, physical therapy and other alternative and complementary treatments are developing quickly.

ABSTRACT

Insomnia is the most common type of sleep disorder in the clinic. Long-term insomnia seriously affects people's work and life, and the incidence of insomnia is increasing year by year.In order to provide reference for the clinic, this paper summarizes the current advances in the treatment of insomnia, including pharmacotherapy and non-pharmacotherapy. Non-pharmacotherapy includes cognitive behavioral therapy and physical therapy. Pharmacotherapy includes benzodiazepine receptor agonists, including benzodiazepine and non-benzodiazepine hypnotics, melatonin and melatonin receptor agonists, antidepressant drugs with sedative effects, antipsychotic drugs, antihistamines that have central inhibitory effects, Chinese medicine.

ABSTRACT

Alzheimer's disease ( AD) is the most common type of dementia. Currently, there is a lack of drugs that could effectively slow and halt the progression of AD. The present review describes the advancements in the treatment of AD, mainly including drugs for symptomatic control, drugs that control the behavioral and psychological symptoms of dementia, and some other drugs for AD and the non-pharmaceutical treatments. It is hoped that early diagnosis, early intervention and comprehensive therapy could relieve the symptoms and slow the progression of AD.

ABSTRACT

Objective To study the application value of parathyrod in situ exposure and protection in differentiated thyroid cancer thyroid carcinoma (DTC) surgery. Methods 500 cases of DTC admitted from Jan. 2012 to Dec. 2014 were selected for the research. According to whether the parathyroid was anatomically exposed during the surgery, the patients were divided into experimental group (exposed group) and the control group (un-exposed group). Then, depending on tumor extent and risk factors, the experimental group were divided into group E1 (lateral affected glandular lobe, isthmus lobe resection and unilateral VI lymph node dissection )and E2 (total or subtotal thyroidectomy and bilateral VI lymph node dissection), while the control group were divided in-to group C1 (surgical treatment same as E1)and group C2 (surgical treatment same as E2). Patients’ postopera-tive nerve and muscle symptoms were observed, such as clinical manifestations of face, lips, hand and foot numb-ness or convulsions. Both preoperatively and postoperatively, patients’ parathyroid hormone (PTH) and serum calcium level indicators were detected before surgery and at the 1st, 3rd, 5th, 7th, 14th and 28th day after surgery. Results The lymph node metastasis rate and number between experimental group and the control group had statistical insignificance (χ2=1.02, 0.79, P=0.14, 0.96). The recurrence rate of the 2 groups had no significant difference during the follow-up (χ2=0.23, P=0.65). Group E1 was significantly lower than Group C1 in postopera-tive parathyroid function decline rate, incidence of hypocalcemia and parathyroid mistakenly cut rate. Group E2 was also obviously lower than Group C2 in all these aspects. The difference had statistical significance (P<0.05). Conclusion Parathyrod in situ exposure and protection assisted by direct vision of the magnifying glass can re-duce the mistakenly cut rate of parathyroid and the temporary hypoparathyroidism, which can effectively avoid the adverse effects caused by total thyroidectomy.

ABSTRACT

Parkinson's disease (PD) is a common degenerative disease of the central nervous system, but no drug has been found to be surely able to protect neurons so far, delay onset or slow progression of the disease. Currently there are a variety of Chinese formulas, single herb medicines, active fractions and monomers showed prophylactic and therapeutic effect on PD animal models. The mechanisms include protection of substantia nigra cells, improvement of neurotransmitter content, anti-oxidation, immune regulation, enhancement of Western medicine efficacy, reduction of side effects, etc. All these mechanisms may play integrated effect and slow disease progression. In particular, Chinese medicine compound may have some advantages in neuroprotective treatment of PD, because a variety of active ingredients can exert multi-links, multi-levels and multi-targets integrated regulation effect on human body. However, the level and standard of Chinese medicine studies on PD animal still need to be improved.

ABSTRACT

Evidence-based medicine mirrors the natural and social properties of human being,harmoniously integrating scientific spirit and humanistic spirit. Discussions were made on such aspects as the relationship between human spirit and harmonious doctor-patient relationship. The humanistic spirit of evidence-based medicine, and the humanistic spirit of evidence-based medicine as used in building harmonious doctor-patient relationship. The authors point out the following: 1) Medical science itself is a wealth of humanistic sprits, and medicinal science can better serve human being with humanistic spirit;2) Evidence-based medicine is completely in line with medical humanities, which scientifically reveals the "People-oriented medicine" as the essential attribute of human sciences;3) Modem clinical medicine has unveiled the evidence-based era. Based on these findings, the authors hold that in such an era,exploration and understanding of the human spirit as the core of evidence-based medicine and focusing on the cultivation of medical humanistic spirit, will be conducive to building a harmonious doctor-patient relationship.

ABSTRACT

Objective To evaluate the extremity function of patients with intracerebral hemorrhage (ICH)using short-latency somatosensory evoked potentials (SEPs) and a modified intracerebral hemorrhage (MICH) scale.Methods On admission, SEP was applied in the examination of 61 patients with ICH. P40 latency and the amplitude of posterior tibial nerve potentials in both the healthy and affected extremities were measured. Abnormalities were classified based on the margin of lower extremity SEP latency and the main waveform changes. MICH was measured simultaneously to prepare a prognosis. The modified Rankin scale (MRS) score was assessed 3 months after the attack as well.Results Compared with the healthy side, there were significant differences in posterior tibial nerve P40 latency and amplitude on the affected side among patients with ICH. P40 latency and MRS scores on the affected side 3 months after the attack were positively correlated. On the unaffected side, P40 amplitude and the MRS score 3 months after the attack were negatively correlated. The MICH score on admission and the MRS score 3 months after the attack were positively correlated. Based on MRS scores (MRS≥4 indicating a poor prognosis), the predictive sensitivities for a poor prognosis of SEP and the MICH scale in patients with ICH were 80.77% and 84.61% respectively, while the specificities were 62.68% and 88.57% respectively, and the accordance rates were 70.5% and 86.9% respectively. Conclusions SEP and the MICH scale are closely correlated with the prognosis for extremity function in patients with ICH.The combination of SEP with the MICH scale might be helpful in predicting the prognosis of the patients with ICH.

AIM: To study the dynamic expression of protease - activated receptors - 1 (PAR- 1 ) after intracerebral hemorrhage (ICH) and the influence of Nao Xue Kang Tablet (NXKT) on it's expression. METHODS: 72 Wistar rats were divided into normal group, ICH model groups (ICH, 6 h, 24 h, 3 d, 7 d), Nao Xue Kang groups (NXKT, 6 h, 24 h, 3 d,7 d).ICH models were produced with the induction of collagenase typeⅦ - S, except normal group. Immunohistochemical method was used to detect PAR- 1 protein and RT- PCR technique was used to detect PAR- 1 mRNA in brain tissues around the haematoma at different time points of different groups. RESULTS: PAR - 1 protein and mRNA were mildly positive in normal group. In ICH model groups, intensity of PAR - 1 expression started to increase at 6 h, and further increased at 24 h. PAR - 1 expression reached the peak at 3 d and began to descend. At 7 d the decent was obvious. At 6 h, 24 h, 3 d, and 7 d time points, the PAR-1 protein positive cell number and PAR- 1 mRNA absorbance ratio in ICH model and NXKT groups were significantly higher than those in normal group ( P <0.05 or P < 0.01). The PAR- 1 protein positive cell number and PAR- 1 mRNA absorbance ratio in NXKT group were significantly lower than in ICH model group ( P < 0.05 or P < 0.01 ). CONCLUSION: After ICH,PAR - 1 is continuously activated because of the stimulation of thrombin. Action of thrombin after ICH may be mediated by PAR- 1; NXKT may inhibit the activation of PAR- 1, so the praxiology is improved. This may be one of the main mechanisms that NXKT could facilitate the recovery of nervous function.
